How can I apply for disability benefits?

To apply online:

• Print and review the Adult Disability Checklist. It will help you gather the information you need to complete the application.
• Complete the Disability Benefit Application.
  
• Complete the Medical Release Form

To apply by phone: Call 1-800-772-1213 from 7 a.m. to 7 p.m. Monday through Friday. If you are deaf or hard of hearing, call TTY 1-800-325-0778.

To apply in person: Visit your local Social Security office. (Call first to make an appointment.)
