To apply online:

Print and review the Adult Disability Checklist. It will help you gather the information you need to complete the application.
Complete the Disability Benefit Application.
Complete the Medical Release Form

To apply by phone: Call 1-800-772-1213 from 7 a.m. to 7 p.m. Monday through Friday. If you are deaf or hard of hearing, call TTY 1-800-325-0778.

To apply in person: Visit your local Social Security office. (Call first to make an appointment.)
